Scott County Government is seeking a qualified Part-Time Community Paramedic to join Georgetown-Scott County EMS (GSCEMS).

The Community Paramedic provides in-home patient care, follow-up services, health education, and care coordination for patients referred by physicians, hospitals, and GSCEMS personnel. The position also assists with overdose follow-up in coordination with the Georgetown Police Recovery Team.

Primary Responsibilities:

• Provide patient assessment, treatment, and follow-up care.
• Conduct post-hospital follow-up, including medication reconciliation, vital signs, EKGs, dressing changes, and blood draws.
• Monitor and document patient conditions and communicate significant findings to physicians and healthcare providers.
• Provide patient education related to chronic conditions, fall prevention, nutrition, and other health needs.
• Coordinate appointments, referrals, and transitions of care with hospitals, physician offices, healthcare providers, law enforcement, and community partners.
• Assist the full-time Community Paramedic with patient caseloads, follow-up visits, referrals, and program activities.
• Participate with PA/APRN providers and qualified mental health professionals in community-based patient assessments and care coordination.
• Assist with overdose follow-up in coordination with the Georgetown Police Department’s Recovery Team.
• Maintain accurate, timely, and confidential patient and program records.
• Support implementation of current community paramedicine best practices and program initiatives.

Requirements:

• High school diploma or GED.
• Associate degree in a medical field preferred. In lieu of a degree, five (5) years of clinical EMS field experience as a paramedic is required.
• Current Kentucky Paramedic Certification.
• Current BLS, ACLS, and AMLS certifications.
• Valid Kentucky Operator’s License.
• Must be eligible for the International Board of Specialty Certification (IBSC) Community Paramedic (CP-C) examination. Candidates will have 12 months from date of hire to complete the requirement.

• Strong patient assessment, clinical, communication, and interpersonal skills.
• Ability to work independently in community settings while collaborating effectively with healthcare and public safety partners.
• Ability to exercise sound judgment and make rational decisions in stressful or emergent situations.
• Ability to maintain accurate and confidential patient documentation.
• Knowledge of EMS protocols, medical equipment, medications, and patient care practices.
• Ability to use or learn ESO, Julota HER, Microsoft Word, and Microsoft Excel.

Position works approximately 20 - 30 hours per week.

Work hours will be coordinated with GSCEMS Administration and the full-time Community Paramedic based on patient needs and program requirements. The schedule may include weekdays and occasional adjustments for patient visits, meetings, special events, public education, and other program activities.
