Position Summary

The Spiritual Care Coordinator serves as a key leader in promoting whole-person healing by integrating spiritual, emotional, and cultural care across inpatient, outpatient, and community settings. Rooted in the hospital’s mission and commitment to patient-centered care, this position supports patients, families, and staff through spiritual care services, coordination of community clergy resources, and collaboration with navigation, care coordination, and patient experience teams.

Core Responsibilities

Spiritual & Emotional Support

Provide spiritual assessments and bedside support tailored to patient preferences and beliefs

Offer prayer, reflection, or ritual appropriate to patient and family needs

Support patients and families during crisis, trauma, serious illness, and end-of-life

Coordinate sacramental or faith-specific needs through community clergy

Patient Navigation & Resource Connection

Serve as a faith-informed patient navigator in collaboration with care coordination teams

Connect patients and families to community, faith-based, and social support resources

Assist with transitions of care and discharge planning for patients with complex needs

Patient Experience Integration

Participate in patient experience rounding and follow-up

Support initiatives that enhance compassion, hospitality, and spiritual environment of care

Contribute to patient satisfaction and quality improvement initiatives

Staff & Community Engagement

Provide spiritual and emotional support for staff including debriefings after difficult cases

Serve as liaison to community clergy and faith organizations

Assist with mission-focused events, reflections, or observances

Administrative & Compliance Functions

Document spiritual care encounters in the electronic medical record

Ensure compliance with HIPAA, Joint Commission, and CMS standards

Participate in interdisciplinary meetings, ethics consultations, or committees as appropriate

Qualifications

Required:

Bachelor’s degree in Theology, Ministry, Religious Studies, or related field

1–2 units of Clinical Pastoral Education (CPE) or equivalent supervised spiritual care training

Minimum of two (2) years of experience in spiritual care, ministry, healthcare, or community service

Endorsement or good standing with a recognized faith community

Preferred:

Experience in patient navigation, palliative care, or mission integration

Bilingual or multicultural ministry experience

Grief support or trauma-informed care training

Physical Requirements

  • Ability to move throughout patient care areas and provide support during emotionally intense situations.
