Position Summary

The Family Nurse Practitioner / Physician’s Assistant provides professional comprehensive primary medical care to SCHC patients, under the supervision of the Chief Medical Officer. The mid-level provider works with other healthcare team members to provide quality patient-centered care in an integrated healthcare setting. The ideal candidate is a Family Practice mid-level who sees both children and adults and is passionate about community-based healthcare access.


Essential Duties and Responsibilities

  • Obtains history and performs physical examination to determine a diagnosis and treatment plan in accordance with established procedures and policies. Requisitions lab tests and x-ray’s, writes prescriptions and makes referrals as deemed appropriate
  • Delegates patient care responsibilities that fall under the scope of other healthcare team members to those individuals (nurse, medical assistant, other clinical or administration staff)
  • Assesses, treats and manages specific illnesses and conditions according to medical regimens, utilizing established protocols
  • Consults and works collaboratively with physicians and other health care providers to develop effective treatment plans. Provides direction to other personnel and reviews medical records to verify that each patient’s care plan and medical needs are being met
  • Provides referral for hospitalization, tests or specialty consultations
  • Provides follow-up care for patients. Ensures continuity of patient and family care by exchanging information and cooperating with other community healthcare agencies, hospitals and healthcare providers
  • Counsels patients and/or families concerning physical and developmental well-being, defining each individuals needs and problem
  • Develops and implements plans for counseling and teaching patients and/or families in the prevention of disease and health maintenance
  • Attends staff meetings and provider meetings, and maintains involvement with SCHC committees to ensure quality of care for patients; participates in development and/or revisions of healthcare protocols; participates in peer review and performance evaluations
  • Participates in quality improvement activities
  • Participates in the delivery of healthcare services at mobile unit sites, including schools, community organizations, and special events.
  • Provides care outside of traditional clinic settings, ensuring access to children and adults across SCHC's outreach footprint.
  • Flexibility is required, as some outreach events or mobile unit visits may occur outside of standard office hours.
  • Engages with staff, community partners, and event organizers to coordinate and deliver seamless care in mobile and embedded environments.

  • Other duties as necessary to ensure successful attainment of SCHC goals and objectives

 

Job Qualifications and Requirements

  • Graduate of an accredited Nurse Practitioner / Physician Assistant program
  • Current Advanced Practice Registered Nurse License / Physician Assistant license in the state of Kentucky
  • Two years or more of professional midlevel experience, preferably in a clinic setting with vulnerable and/or diverse population
  • CPR/BLS Certificate
  • Previous professional experience working with pediatrics is strongly preferred.
  • Attend/complete CME annually as directed by the Kentucky State Board of Licensure
  • Knowledge of professional theory and practice to give and evaluate patient care
  • Knowledge of medical equipment and instruments to administer patient care
  • Skill in applying the principles, methods and techniques to provide on-going patient care
  • Skill in identifying problems and recommending solutions
  • Skill in preparing and maintaining records and writing reports
  • Skill in developing and maintaining department quality assurance
  • Skill in establishing and maintaining effective working relationships with patients, medical staff and the public
  • Ability to react calmly in emergency situations
  • Ability to interpret and apply guidelines and procedures
  • Ability to work in a team-oriented environment
  • Strong communication and interpersonal skills
  • Knowledge of community needs and initiatives and/or experience working in a community health setting
  • Preferred knowledge of local and regional resources for low-income and at-risk populations
  • Respect for and appreciation of diversity

  • Work is performed in SCHC clinics and mobile unit.
