The Family Practice Physician provides comprehensive primary care services to patients of all ages in an outpatient setting. This role includes preventive care, acute care, and chronic disease management while supporting population health initiatives, quality improvement efforts, and the Center’s mission to reduce health disparities. 

Essential Duties and Responsibilities 

Provide comprehensive primary care to pediatric, adolescent, adult, and geriatric patients 

Diagnose and treat acute and chronic medical conditions 

Deliver preventive care services, including well-child visits, immunizations, screenings, and health education 

Develop, implement, and manage individualized treatment plans 

Order, interpret, and evaluate diagnostic tests and laboratory results 

Prescribe medications and therapies in accordance with evidence-based guidelines 

Document patient encounters accurately and timely in the electronic health record (EHR) 

Collaborate with interdisciplinary care teams, including behavioral health, dental, pharmacy, and care coordination staff 

Participate in quality improvement initiatives, population health management, and value-based care activities 

Ensure compliance with clinical policies, HRSA requirements, CMS regulations, and HIPAA standards 

Participate in team meetings, trainings, and professional development activities 

Provide supervision and teaching to medical students, residents, or advanced practice providers, as applicable 

Required Qualifications 

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school 

Completion of an accredited Family Medicine residency program 

Board Certified or Board Eligible in Family Medicine 

Current, unrestricted Kentucky medical license (or eligibility for licensure) 

Current DEA registration 

BLS certification (ACLS preferred)
